A young lady of 29 years old, with no background of medical history, was referred to you by GP due to around 7-8 months history of intermittent abdominal pain and diarrhoea. On further questioning, she did have intermitent abdominal complaints since 4 years ago.  She felt her abdominal pain (generalised abdominal pain, crampy in nature, intermitent) was worsening around 8 months ago, when she took up new jobs. She is also complainted of having intermittent diarrhoea of around around 3-4 day per week (around 5-6 episodes per day). No nocturnal symptoms, no bloody diarrhoea. There is no upper GI symtptopms, no haematemesis, no malaena, no reduced in appetite, no weight loss.  All her blood tests including full blood count, renal function, liver fducntion, inflammatory marker were all normal. What is your working diagnosis ?? What will be the investigations required ??

I would diagnose this as a case of Irritable Bowel Syndrome or IBS, which is defined as a disorder that is characterized by lower abdominal pain or discomfort, diarrhea, constipation, gas, bloatedness, and nausea. This condition can be diagnosed clinically using the Rome Criteria which states that symptoms must be present at least 12 weeks (which need not be consecutive) in the preceding 12 months, of abdominal discomfort or pain that has two out of three of these features:
