The challenges in managing chronic disease - Compliance

Many of us do encounter patients with chronic disease like type 2 diabetes, hypertension, hypercholesterolaemia on a daily basis. Some of the patients are not compliance in taking their daily medications, despite multiple advises given, results in uncontrolled high blood pressure or high blood sugar level / elevated HbA1C, which can be quite risky to those patients cohort. What will be your strategy in dealing with this issues ?
