On a daily basis, hospital pharmacists are faced with many different challenges. I am sure that the mistake of the pharmacist in Canada mentioned in this article was purely unintentional. However, it also reflects negligence on her part and even caused death in the child who ingested the formulation. For the pharmacists in the MIMS community, do these things happen in hospital pharmacies in your respective countries? I have not yet heard of any occurence like this in the Philippine hospital set...
