As nurses make up the largest proportion of care providers, they are most likely to make medication errors. They are potentially responsible for committing medication errors as high as 50 – 80% prior to reaching the patient.

This issue seem to be a nightmare for our part. In my 3.5 year of practice, I am guilty for commiting an error especially in medication. I think almost all nurses can be guilty of this and/or there are just one in a million chances that there could be a nurse who has not commit a single error.
